E-MAIL CONSULTATION Your Patient Advocate will provide you with a health screening questionaire. review the questionaire with you to clarify your concerns. research your health issues and provide you with health information & local resources. $60. for Up to 30 E-mails  from Your Patient Advocate

TELEPHONE & E-MAIL CONSULTATION Your Patient Advocate will provide all the same service as the E-MAIL CONSULTATION. with the addition of one hour of telephone discussion, can be up to six calls. including a hard copy of the research and findings mailed to you first class. $150 for up to 30 E-mails & 60 minutes of Telephone Time from Your Patient Advocate.

IN PERSON CONSULTATION Your Patient Advocate will visit with you for one hour at your location or ours. provide all the services included in the TELEPHONE and E-MAIL CONSULTATIONS.  assist you in developing an action plan with your doctors and other health providers. $200. per hour
